BACKGROUND CHECKS
There are always questions regarding background checks for parents attending field trips with their child(ren). The Diocese's policy states the following:
"Any volunteer who chaperones a field trip or overnight stay on even one occasion is required to complete safe environment training and a background check."
Therefore, if someone volunteers for a field trip, regardless of whether they will have sole supervision of students or not, they will need to have safe environment training and a background check done prior to the field trip.
As of 10/1/24, background checks cost $19 at the applicant's expense and last for 5 years. To check on the status of a background check, please reach out to the Business Manager. Background checks must be paid for in the school office (checks payable to Holy Family School) - the applicant will NOT be charged online when they apply for the background check.

PERMISSION SLIPS
STEP 1: Signed permission slips are required from ALL students prior to leaving the building. There are two types of field trips - field trips that stay local (in town) and field trips that go out of town. Please download the appropriate permission slip below, complete and turn into the Main Office for copies (if needed). (NOTE: there is one in-town permission slip that should be used when you are looking for parents to sign up to be chaperones.) Each teacher must track permission slips for their own students and completed permission slips should be kept in the classroom for the duration of the school year.
STEP 2: In addition to safe environment training and background checks (see above), any adults attending a field trip (regarless of whether it is in or out of town) must complete an Adult Liability Waiver.
STEP 3: Once you have confirmed your field trip date, send an email to the office AND kitchen staff informing them of the date. Let them know if your class/grade level will be eating lunch on the day of your field trip so that the cooks can plan appropriately (this should be done at least 2 weeks prior to the field trip).
STEP 4: If your field trip will require bussing, you must submit a Transportation Request to the Main Office via the "Bus Request Form".
STEP 5: Calculate what the cost will be for your field trip so that you know how much to charge each student. You can use the cost calculator below. IMPORTANT: Only enter information in the red boxes - the costs will automatically calculate for you once you input this information.
